Athletic forms and documents

Forms must be on file in the Athletics Office in order for a student-athlete to be eligible to participate in any MUHS sport. Students CANNOT participate in tryouts unless the updated forms are submitted.

if a student-athlete has not had a sports physical in the past two years, one will be required before the student-athlete will be allowed to participate in practice or competition. The physical must be after April 1, 2024.

Download and complete these two forms. Return to the Main Office, Attn: Angela Balistrieri. Please do not give forms to coaches.

The forms listed below will need to be completed and submitted online via Arbiter Registration, the new online system. Information about how to register is available below. 

  • WIAA Eligibility Information Form
  • Athletic Code of Conduct
  • FERPA (Family Education Rights and Privacy Act of 1974)
  • Concussion and Sudden Cardiac Arrest Waiver

Arbiter Registration Account

New this year, we are now using Arbiter Registration to collect all athletic forms and documentation. (This replaces the previous online system.) ALL families with students interested in participating in any sport, must create an Arbiter Registration account. No information has been carried over from the previous online system. Directions are outlined below.

Please follow these steps:

  1. Create an Arbiter Registration account (you only need one account for your whole family).You may create your account by clicking here.
  2. Activate your Arbiter Registration account using the activation link that will be automatically sent to your email. (Note: this email may go into your junk folder). If you are having trouble creating your account, receiving the activation email, or logging in, you may contact support at registration@arbitersports.com or call 800-311-4060.
  3. Complete your athletic registrationRegistration Link
  4. Note: Click any sport that your son maybe interested in. You have to pick at least one sport for the whole school year.

Health Insurance Coverage

Anyone participating in interscholastic athletics or competitive club sports must have health insurance. All athletes must be protected with personal health insurance in case of injury. Marquette University High School does not provide health insurance coverage for athletes. In any sport, especially contact sports, there is a risk of injury. Injuries to organs, paralysis and even death may occur. Students without health insurance may not practice or compete in any sport.

If a student would like to participate in a sport and does not have insurance, Marquette High has information regarding a school-year policy provided by K & K Insurance where the family is able to purchase coverage for the school year.
